I (we) agree as follows:

1) Mandatory execution of form:
I acknowledge that my execution of this waiver is a prerequisite to participate in the Asha Beyond Borders Fitness Challenge.

2) Voluntary Participation:
I acknowledge that I have voluntarily applied to participate in the Asha Beyond Borders Fitness Challenge. I certify that I am in good health and fully capable of participating in this event. I also acknowledge that I am medically able to do so and am properly trained for the physical activity being undertaken.

3) Publicity Waiver:
I hereby grant permission to Asha for Education (referred Asha here in after) to use any photographs, motion pictures, recordings or any record of this event for any legitimate purposes.

4) Assumption of Risk:
I AM AWARE THAT PARTICIPATING IN THIS EVENT INVOLVES PHYSICAL ACTIVITIES WHICH ENTAILS RISK, INCLUDING INJURIES, OR, IN EXTREME CASES, EVEN DEATH. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH.

5) Release:
In consideration of your accepting my entry to the event, I, intending to be legally bound, do hereby, for myself, my heirs, executors, and administrators, forever waive, release and give up any and all claims, demands, liability, damages, costs and expenses of any kind whatsoever, including personal injuries to me or wrongful death against the organizers and sponsors of the event (RELEASED PARTIES).

6) Compliance with Children’s Online Privacy Protection Act (COPPA):
I acknowledge that, in compliance with COPPA, I’m over thirteen (13) years of age, and that if I register a child under fourteen (14) years of age I’m the parent of such child, and do hereby consent to the collection of such child’s personal information by Asha.

7) Compliance:
As a participant in the Asha Beyond Borders Fitness Challenge, I will respect the requests of others, and follow the rules that are established by the event organizers and volunteers.

8) Validity, knowing and voluntary execution:
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I fully understand the respective intent and meaning of all of the terms and provisions hereof. I understand that by registering for this event, I agree to follow the conditions above. Should any portion of this waiver be judicially determined invalid, voidable or unenforceable, for any reason, such invalidity shall not affect the remaining portions of this waiver. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, AGREE THAT I HAVE CAREFULLY READ AND FULLY UNDERSTAND ITS CONTENTS, AND SIGN IT OF MY OWN FREE WILL FOR MYSELF AND/OR FOR MY CHILD without coercion or duress.

By registering for the event, I (we) agree to the above waiver and also agree that I (we) am 18 years or older.
